The USS Form Generator is mainly designed to create web based forms to collect data for later use.  Therefore, by designing your web poll using the Form Generator, you gain the ability to analyze and manipulate the data gathered by your form at any time!

The main drawback to using the Form Generator to create web poll is that it cannot be embedded into an existing web page.  You must provide a link from your web page to the web poll.  An example poll can be found by clicking here.

Results from a poll created by the Form Generator can be obtained only by you, as you are the sole owner of the poll.  You can choose to make the results public if you wish.

To create your own web poll using this system, follow these steps.  Please note that full documentation on how to use the Form Generator is available by clicking here [PDF Format].

1. Click here.  This will take you to the Form Generator homepage, which can be found at http://uss.tcnj.edu/formgen.
2. Enter your valid TCNJ UNIX account name into the blank provided.  If you do not have a TCNJ UNIX account, please visit the Help Desk in Green Hall room #6 to obtain one.  Click the "Continue" button when you have finished.
3. If you have used the Form Generator before, enter in your Form Generator password.  If you have not, then create a password for yourself.  If you have forgotten your password, please contact the system administrator, Craig Kapp, at kapp@tcnj.edu.  Click the "Continue" button.
4. Choose "Create new form" from the Main Menu.  Click the "Continue" button.
5. Follow the prompts on the next few screens to generate a poll.  Refer to the documentation if you get confused.
6. When you have finished, the Form Generator will return a link.  Upon clicking this link, you will be transported directly to your poll.  To add this poll to your website, simply create a link from the desired page.  How do I insert a link into my website?

Once you have a working poll, you may feel the need to modify it.  To do this, follow these steps:

1. Click here.  This will take you to the Form Generator homepage, which can be found at http://uss.tcnj.edu/formgen.
2. Enter your valid TCNJ UNIX account name into the blank provided.  Click the "Continue" button when you have finished.
3. Enter in your Form Generator password.  Click the "Continue" button.
4. Choose "Modify existing form" from the Main Menu.  Click the "Continue" button.
5. Change whatever you need to change on your form.
6. Once your form has been re-published, you're finished!  There's no need to create a new link from your web page(s) - the existing link will work just fine.
